Dental Coverage Options

***SCOPE OF COVERAGE/EXCLUSIONS : Coverage includes general dentistry and listed surgical services only. NO endodontic treatment (root canals), open-and-med procedures, or related emergecy endodontic services are offered or performed.

Pricing & Coverage Rates

Full Day: 8 Hours
Half Day: 4 Hours

Overtime: Additional time beyond scheduled hours billed at $150/hour
 

Short Notice Priority Coverage Service

Urgent Scheduling Needs? Coverage may be available on short notice subject to a priority coverage fee based on availability

 

Bookings made within 48 Hours: + $150

Bookings made within 24 Hours/Same Day: + $250
 

Booking Policy

A $100 non-refundable deposit is required to reserve your requested date. This deposit will be applied toward the total balance due.
 

Example – Full Day Booking ($1,200):

$100 deposit due at booking

$1,100 balance due by the end of the scheduled 8-hour workday
 

Cancellation Policy

More than 7 days notice: Deposit may be transferred to a future coverage date, subject to availability.

Less than 7 days notice: Deposit is forfeited.

Credentialing & Compliance

For your convenience, all required professional documentation will be provided after booking to support a smooth onboarding and credentialing process, including:
 

  • National Practitioner Data Bank self-query report
        *

  • Active Dental License

  • DEA 

  • CPR/BLS Certification

  • Malpractice Insurance Verification

  • Government-Issued Identification

  • Additional Credentialing Documents as Needed
